Consent To Treat Minor
A new Tennessee state law requires that we have written or verbal consent from a parent or legal guardian to treat any unaccompanied minor or any minor that is brought in by someone other than the legal guardian. This includes sick appointments, lab work, and other treatments.To ensure that the child receives the care they need, parent or legal guardian must complete the consent form which authorizes us to treat the minor child accompanied by someone other than their legal guardian.
